急，孩子的LTVP要交的材料的问题？求问

各位：
我孩子的LTVP已经原则批准了。我看到上面一些需要的递交的材料。
1、FORM14
2、一张彩色照片
3、申请人的护照COPY
4、DE CARD
5、申请人的出生证明
6、我的NRIC
9、结婚证明
就以上东西，还缺其他材料吗？比如我的收入证明和孩子要不要体检啊？
请指点一下。谢谢啊!

Duly completed Form 14* signed by the applicant and a local sponsor (the local sponsor must be either a Singapore Citizen (SC) or Singapore Permanent Resident (SPR)
Duly completed Form V39I* signed by the local sponsor (Malaysians are not required to complete this form)
Applicant's travel document (e.g. passport) with at least 6 months validity. A copy of the travel document personal particulars page must be submitted to ICA
Applicant's Disembarkation/Embarkation (D/E) card with a valid Visit Pass granted on entry into Singapore
Parents' Identity Cards (copies of the NRIC must be submitted to ICA)
Applicant's Birth Certificate
Local sponsor's Marriage certificate (original and photocopy); if the local sponsor is divorced, the local sponsor's Divorce Certificate and Custody Papers; if the local sponsor is widowed, the spouse's Death Certificate
If only one of the parents is a SC/SPR:
Local sponsor's Income Tax Notice of Assessment for the last 3 years. Alternatively, the sponsor may choose to completeAppendix (VP) to give consent for ICA to obtain and verify financial information provided in respect of this application with the Inland Revenue Authority of Singapore (IRAS) directly instead.
Local sponsor's CPF Statement showing monthly CPF contribution for the last 12 months
Local sponsor's Letter of Employment stating Date of Commencement, Designation and Salary per month (for self-employed: sponsor's valid Registration Certificate from the Accounting and Corporate Regulatory Authority (ACRA website)and Company's Profit & Loss Statement for the last 12 months)
Local sponsor's highest Educational Certificate
One recent passport-sized colour photograph (to be pasted on the top right-hand corner of Form 14)
