请教给孩子申请公民

我刚刚做完三部曲，我想问是不是拿到小纸条就可以给小孩子申请公民了？如果是，在ICA网站上有写如果还是不是在新加坡出生，父亲需要作出法定声明述明该（妻子的名字）是他的一个和唯一的妻子。这个声明怎么做？是自己写一下就可以还是要找什么公证，律师之类？
Marriage Certificate. If parents’ marriage was registered outside Singapore, child’s father is required to make a Statutory Declaration stating that (name of wife) is his one and only wife if he is the applicant for child’s citizenship.

那个出生证应该无所谓，可以忽略

我孩子是在国内出生的，我申请公民的时候还没有出生。我去给我孩子申请的话用哪个表格？
还有是UNDER For children of Singapore citizen(s) by registration Child’s Documents
还是 UNDER For children of Singapore citizen(s) by descent
刚才一直在ICA看迷糊啊！
