After received the letter from ICA, you must to complete the PR formalities within 2 months. You need to book appointment again via ICA website and bring along the documents as below,

1. IPA letter
2. Passport with work pass
3. Acknowledge letter from CPF (online submit via CPF website with singpass)
4. Medical report.
5. 2 colour passport size photos
6. EP152 form (Employment certification)
7. Form 6 (Permit to re-enter Singapore)
8. Form of IC registration

好的，这个详细。问三个问题啊:
1. 这些form和CPF的材料都要去哪里找呢？
2. 有一个medical report，也就是还要做次体检?
3. 直到拿卡，前后大约要多少花费呢？

** u need to make payment when submit the form to ICA, should be around $150-$200, will keep you updated after i submit next week :)
